Forms can either be posted to us or emailed to hwccg.haresfield@nhs.net
If you would prefer to speak to an appropriate member of staff to complete the form over the phone together then please contact the practice to state this including your availability Monday- Friday between 8- 5 pm in the next 14 days.

When we are closed.
GP Out of Hours.
If you need urgent attention or advice please telephone 01905 422883 or 01905 616977 and an answer phone message will give you the number of the GP Out of Hours Emergency Service.
Please do not ask to see a doctor out of hours unless you genuinely cannot wait until the surgery re-opens.
999
Call 999 in an emergency. Chest pains and/or shortness of breath constitute an emergency.
NHS 111.
111 is the NHS non-emergency number. It’s fast, easy and free. Call 111 when you need medical help fast but it’s not a 999 emergency.
You will speak to a highly trained adviser, supported by healthcare professionals. They will ask you a series of questions to assess your symptoms and immediately direct you to the best medical care for you.
NHS 111 is available 24 hours a day, 365 days a year. Calls are free from landlines and mobile phones.

Enhanced Access Appointments.
We now offer Enhanced Access appointments with a GP (subject to availability). These appointments are available outside our usual Practice opening hours and are available on weekends.
The appointments will be telephone consultations, meaning you don’t have to travel anywhere to receive care from our GP.
These appointments are pre-bookable by contacting the surgery during our opening times – when calling please ask for an Enhanced Access appointment and our receptionists will be able to help you.
Accessibility.
All medical facilities are fully accessible for disabled patients, including the lifts to the first floor.
